Manitoba offers the Self and Family Managed Care Program to eligible home care recipients. The program offers funds in exchange for home care services which can be used to hire private care or a care company such as Comforts of Home – Care.  By going this route families and individuals will  have a say about who comes into their home ensuring  consistency and quality of care.

 

How Self and Family Managed Care (SFMC) Works

The Self and Family Managed Care (SFMC) program allows you to manage your own home care services while receiving funding support. Here’s how the process works:

1. Eligibility
Individuals must live in their own home, apartment or assisted living facility.

2. Apply for the Program
Ask your existing home care case coordinator about SFMC or phone the Intake Line directly at (204) 788-8330 to apply.

3. Complete the Application
Complete the application form.  Comforts of Home – Care can assist with the application form.

4. Care Assessment
Your home care coordinator will assess how many hours of care and individual is eligible for.

5. Set Up a Dedicated Bank Account
If approved individuals will be required to open a separate bank account to manage the funds.

6. Receive Funding
The program is subject to regular audits, so it is important to keep all invoices and documentation.

7. Ongoing Regional Health Authority Support
Your Regional Health Authority (RHA) will continue to provide medical equipment, supplies (such as gloves and pads), and visits from nurses and Community Therapy Services.

Regional Health Care home care services suggest families and individuals have a  back-up plan as the services they provide are not always guaranteed.
